Individual Program Plan (IPP)

A plan of action detailing an individual’s current situation, needs and status combined with identifying goals across all facets of the person’s life. This is often written by a case manager with support and context provided by the individual’s family and medical providers. Rehab Care’s version of this is called a Home Assessment.

Individual Education Plan (IEP)

A plan or program developed to ensure that a child with an identified disability who is attending an elementary or secondary educational institute receives specialized instruction and related services. Often written by an educational advocate/attorney with support from case managers and medical providers.

In Home Support Services (IHSS)

A California program designed to provide pay for either a third party or family member to provide homemaker and personal care assistance who have a low income and need help to remain independent at home. Services provided must be domestic in nature and include things like bathing/grooming, housecleaning, dressing, meal preparation and laundry.
