Our Nurse Case Managers/Care Coordinators facilitate and assist with placement in the following residential environments:
Rehab Care will come to your location and be there to conduct a full assessment of your current living situation. We will provide then our recommendations and suggestions to ensure that the Patient not only is comfortable in their new home, but has all their medical needs met.
